CFI Care Management
It’s About Choice
Our program offers an alternative to nursing home placement for those choosing to remain in their own homes. CMCC case managers work with individuals and families to promote independent living through person-centered, cost-effective service plans.
Collaborative Case Management
Professionals from our case management team meet clients where they live to assess need. We work with the individual and their family to create a person-centered service plan. Based on this plan, CMCC case managers then coordinate services provided by local agencies, government programs, or community organizations to supplement care given by family, friends, or neighbors.
An Ongoing Relationship
CMCC case managers monitor services and resolve concerns to ensure quality. We advocate for clients in need of resources (e.g., home modifications, medical equipment, in-home services) to ensure independence, safety, and quality of life.
Thanks to our affiliation with Crotched Mountain Foundation, CMCC has grant funding available, which can offer clients additional resources including medical equipment and home modifications not covered by insurance as well as items needed for basic needs.
We can support a variety of needs, including:
- Nursing care
- Home health aides (LNAs)
- Homemakers (housekeeping)
- Adult medical day care
- Personal emergency response systems
- Respite care
- Medical transportation
- Prescription drug programs
- Housing resources
- Mental health resources
- Durable medical equipment
- Assistive technology
- Referrals to community resources
- Nutrition programs (Meals on Wheels, Commodity Foods)
How to Get Started
The first thing to do is contact ServiceLink to see if you are elibible for the Choices for Independence (CFI) Medicaid waiver. To find your local ServiceLink click here.
Individuals who are eligible for the CFI waiver (formerly known as the Home and Community-Based Care program for the Elderly and Chronically Ill, or HCBC-ECI) may be able to obtain services at no cost.
